Which is the most efficient way of including functions to be used which you have written. And also classes.

Should I be keeping a library with all the functions and classes which is included on every page, or including each function by itself when it needs to be used?

IMHO
i create some like library for SQL, template, common,
and than need including in project.....
in some situation people create shortest lib and use in other project....
